CFC Therapy accepts the following insurances and offers a sliding scale if needed.

  • BCBS PPO (In-Network)
  • BCBS Blue Choice (In-Network, All Metallic Plans)
  • United Health Care (Select Providers)
  • AETNA (Out-Of-Network)
  • Magellan (Out-Of-Network)
  • Cigna (Out-Of-Network)
  • Amita HMO (In-Network)
Great news…a lot of the BCBS choice plans do not require individuals to meet their deductibles prior to receiving mental health benefits. This means that in many cases clients will only owe a copay or coinsurance. Please fill out the benefit and verification form below and we will do the work for you to help you better understand your plan and use your insurance effectively.

Benefit Verification Process

As a reminder, all of our providers are in-network with BCBS, PPO. Emmi and Crystal are the only providers in-network with United Healthcare. At this time we do not accept any HMO’s.

We will call you back at our earliest convenience to take your insurance information.

Cancellation Policy

If you need to cancel your appointment, we require a minimum of 24-hours notice; otherwise, you are subject to a $100 charge for the missed appointment. Messages may be left on the voicemail which will accurately record the date and time you called.
